Reimbursement / Check Request Form

Please confirm there are funds in your budget before submitting a request. Budget balances and financial reports are on the HSC Documents & Resources page.

Requests to go over budget should be sent to Louise@osterhsc.org.

If you have several paper receipts, you may place them in a sealed envelope addressed to Treasurer in the black HSC treasurer box in the teacher workroom.
